Diabetes Educators are an important link between you and the other members of your health care team.

A Credentialled Diabetes Educator (CDE) is a health professional who is an expert in diabetes education and management. They can support you in self-managing your diabetes, overcoming challenges, and living well with diabetes.

Our Diabetes Educators support people with newly diagnosed diabetes, living with Type 1, Type 2 and Gestational diabetes, as well as those at risk of developing diabetes.

Elizabeth Kinnersly (Credentialed Diabetes Educator/Nurse Practitioner)

Liz did her initial registered nurse and critical care training at Ballarat Base Hospital and left to work in Brisbane, Glasgow and Melbourne. Liz has been working as a diabetes nurse consultant in Melbourne at Western Health (strong gestational diabetes focus) and Alfred Health (transplant and steroid induced diabetes) and more recently as a diabetes nurse practitioner in primary care in Geelong and Tasmania. Her special interest is decreasing complications of type 2 diabetes through tailored medications.

Liz is a Credentialed Diabetes Educator (CDE) who enjoys working with adults to help them understand and manage their condition and to live well with diabetes every day. CDEs are recognised by the Australian Diabetes Educators Association (ADEA) for their specialised knowledge and professional development in the field of diabetes education.

Liz is also a Nurse Practitioner (NP) with a Master of Advanced Nursing Practice (Nurse Practitioner) allowing advanced practice in diabetes education and management, and she specialises in insulin and injectables initiation and dose titration for Type 2 diabetes in a primary care setting. Liz can work with established Type 1 diabetes and Gestational Diabetes (GDM) liaising with endocrinology.

Li Yen Hoo (Credentialed Diabetes Educator/Accredited Pharmacist)

Li Yen is a qualified Credentialed Diabetes Educator, health coach and accredited pharmacist and has been working as healthcare professional for over 18 years.

She is passionate about providing personalised diabetes education and care that empowers people with diabetes to deal with their ongoing daily self-management. Li Yen has a special interest in diabetes technology and is a certified trainer in insulin pump therapy and continuous glucose monitoring systems.

Li Yen can speak English, Chinese (Cantonese), Chinese (Mandarin) and Malay.
